SSRI: geneesmiddel bij long covid

Omdat meerdere mensen naar de werking vroegen van een SSRI (serotonine heruptake remmer) bij long covid (= Postcovid), hieronder de mogelijke zes werkingsmechanismen van SSRI bij long covid op een rij. Met daaronder de literatuurverwijzingen.
Let wel: een SSRI wordt bij long covid voor andere werkingsmechanismen ingezet dan voor een depressie!

– 1. Bij long covid is de hypothalamus-hypofyse-bijnier-as (HPA-as) gestoord. Hierdoor hebben mensen met long covid slechts 50% van hun normale cortisolspiegel. Het hebben van genoeg stresshormoon cortisol is nodig om overdag genoeg energie te hebben om te kunnen leven. Een SSRI herstelt deze HPA-as meer of minder.

– 2. Verder vermindert een SSRI bepaalde cytokinen in de hersenen (boodschappers tussen afweercellen) waarvan het niveau bij long covid verhoogd is. Het werkt via de sigma-1 receptor (ontvangt neurologische boodschappers in de hersenen). Ook hierdoor gaan mensen met long covid die een SSRI krijgen zich beter voelen.

– 3. Een SSRI heeft een gunstig effect op de verstoringen in het autonome (onwillekeurige) zenuwstelsel. Zoals middels invloed op pontiene kernen als onderdeel van het reticular ascending system (formatio reticularis). Daarom kan een SSRI ook een rol spelen in de behandeling van dysautonomie bij PostCovid.

– 4. Een SSRI kan de overprikkeling verminderen. Die ontstaat namelijk door derealisatie (onderdeel van de depersonalisatiestoornis). Hierbij werken de unimodale (afzonderlijke) sensorische hersendelen (bijvoorbeeld de perceptie van een steen uit een muur) minder goed samen met de associatieve (samenhangende) sensorische hersendelen (waardoor de hele muur wordt waargenomen). In plaats van gelijk een muur, zie je dan eerst alle stenen apart. Dat is natuurlijk doodvermoeiend. Uit ervaring en onderzoek weten we dat een SSRI daar soms goed tegen kan helpen.

– 5. Een SSRI verlengt de stollingstijd (mogelijk potentieel goed om microclots op te lossen)).

Er zijn dus zes mechanismen waarom een SSRI zou kunnen werken bij long covid:

– 1. Immunologisch (vermindering van bepaalde cytokinen in hersenen). Dit is meerdere keren aangetoond (via sigma-1 receptor).

BELANGRIJK:
fluvoxamine is agonist van de Sigma1 receptor
– Sertraline is antagonist
– Paroxetine is geen aanzienlijke werking op de Sigma1 receptor.
– Citalopram en fluoxetide lijkt op dit punt op fluvoxamine.
Dus de voorkeur gaat uit naar fluvoxamine, fluoxetine en citalopram! Venlafaxine geeft vaak teveel bijwerkingen bij long covid patienten.

– 2. Invloed op hypothalamus-hypofyse-bijnieras. Een SSRI herstelt deze as (gedeeltelijk). Long covid patienten hebben maar de helft van de normale hoeveelheid cortisol vergeleken met gezonde patienten door hun gestoorde as. Literatuur:

– 3. Invloed op autonome zenuwstelstel via hypothalamus en hersenstam (zie literatuur onder 3). Een ssri herstelt (deels) de dysautonomie bij long covid patienten. (zie literatuur onder 3.)

– 4. Klinisch is er sprake van overprikkeling, wat fenomenologisch gelijkt op derealisatie (onderdeel van de depersonalisatiestoornis). Dan kunnen de primaire sensorische hersendelen de associatieve hersendelen niet meer goed ‘vinden’ (ze zijn dan gedissocieerd). (Dan zie je bijvoorbeeld niet eerst een muur, maar eerst alle stenen en dan pas de muur). Dat is zeer vermoeiend en daar raak je overprikkeld door.
Ook hiervoor wordt zowel in de psychiatrie als de verslavingszorg (als er een dissociatieve drugs ingenomen is, waarna de depersonalisatiestoornis niet verdwijnt) een SSRI voorgeschreven.

I advise long covid patient an SSRI. That’s an serotonine reuptake-inhibiter and normally used for depression and anxiety. In this case its works in another way! See the workingsmechanismes below. We/ I have now 300 patients we advised on this. 60 á 70 % react positively (more or less). For the group who doesn’t react on an SSRI, we give an antihystaminicum and N-acetylcysteïne. That works more or less in this group of patients.
We hardly need an RCT (double blind research) of SSRI with long covid patients!

– 1. In long covid (postcovid), the hypothalamus-pituitary-adrenal axis (HPA axis) is disturbed. As a result, people with long covid have only 50% of their normal cortisol levels. Having enough stress hormone cortisol is necessary to have enough energy during the day to live. An SSRI restores this HPA axis more or less.

– 2. Furthermore, an SSRI reduces certain cytokines in the brain (messengers between immune cells) whose level is increased in long covid. It works through the sigma-1 receptor (receives neurological messengers in the brain). This also makes people with lung covid who receive an SSRI feel better.

– 3. An SSRI has a beneficial effect on the disturbances in the autonomic (involuntary) nervous system. Such as through influence on pontine nuclei as part of the reticular ascending system (formatio reticularis). Therefore, an SSRI can also play a role in the treatment of dysautonomia in PostCovid.

– 4. An SSRI can reduce overstimulation. This is because of derealization (part of the depersonalization disorder). In this case, the unimodal (separate) sensory brain parts (for example, the perception of a brick from a wall) cooperate less well with the associative (cohesive) sensory brain parts (so that the entire wall is perceived). Instead of a wall right away, you first see all the bricks separately. That is of course exhausting. We know from experience and research that an SSRI can sometimes help against this.

– 5. An SSRI prolongs the clotting time (potentially good to dissolve microclots)).
